Middle School Sports

 

Practices run from 4:15 PM - 5:30 PM.  All students MUST be picked up by 5:45 PM.

For the safety of our student athletes, all sports and activity practices are closed to families.

Please submit ALL completed paperwork before the first day of practice.  All participating students must turn in a current health exam on file.

 

When registering to participate, please complete the following forms:

• Health Exam form signed by a doctor, student, and parent/guardian. Health exams are good for 18 months and must be current throughout the season of the sport.
• Middle School Activity Participation Form completed by parent and student to give consent to participate in activities.‌
• Paperwork should be turned in to the AAFP, Ms. Russell, by the MPR.
