Clark Middle School Registration
Online registration for returning students Returning students can be registered May 4 - 29 and July 22 - Aug. 7 through Zangle ParentConnection for the 2015-16 school year. Skip the lines and register your child(ren) from the comfort of your own home. Your child is eligible if returning to the same school, or continuing on to his/her neighborhood middle or high school. New-to-district students or students moving over the summer will need to register in-person at their school during districtwide registration in August.
Parents of middle and high school students can register online to save time, but students will still have an opportunity to visit their school to pick up class schedules, locker combinations and parking permits, and to get ID photos taken. All students must register prior to the beginning of school. If you need assistance determining the school your child will attend, please use our online boundary tools or call Demographics at 907-348-5190. Anchorage School District (registration form)
This is required to register all students for the school year. Returning ASD students DO NOT need to complete this form. They will receive a pre-printed version at registration.
Release and disclosure of directory information

Survey permission form
This form regards parent/guardian consent for students to participate in certain school surveys.
Media release form
Used to grant parent/guardian permission for use of student information, photos, images, etc. for use in media projects.
Internet agreement for students
Sets out guidelines and permission for use of the district's Internet equipment and systems.

Parent permission form allowing for a responsible, trained student to carry and/or self administer prescription labeled medication for asthma, severe allergic (anaphylactic) reaction, or diabetes.
Long Term Non-Prescription Medication Request
Parent permission form for requesting the district give non-prescription medicine under certain conditions.
Long Term Request for Administration of Prescribed Medication Form 317B
This form or a written statement signed and dated by the physician is required for medication prescribed for more than 15 days.
